CSR Nutrition didn’t start as a big idea,
it came from working inside facilities and seeing how things were actually being done.
A lot of the time, dietitians were there to complete documentation, but not always fully integrated into the team or the decision-making process. There were gaps in communication, follow-up, and consistency, especially with high-risk residents.
Over time, it became clear that the issue wasn’t effort it was lack of structure.
So instead of continuing to work around those gaps, we built something that addressed them directly.

With meticulous attention to detail and a commitment to excellence, we didn’t build a service. We built a system.
CSR Nutrition was built from a gap we saw daily in long-term care:
nutrition services were being treated as tasks not as clinical systems.
Dietitians were placed to complete notes, not to influence outcomes.
Compliance was reactive. Communication was fragmented.
And critical issues like significant weight loss were often identified too late.
So we restructured the model.
We integrated dietitians into the interdisciplinary team with clear clinical ownership, standardized workflows, and real-time oversight.
From QAPI reporting to high-risk tracking, from admissions to follow-ups every component was designed to function as part of a coordinated system, not isolated tasks.
